This questionnaire is designed to establish likely eligibility for long-term care services and supports available to individuals with disabilities ages 18-64 who are receiving MassHealth Standard. Most (but not all) services and supports covered by this questionnaire are also available to qualified individuals receiving CommonHealth. This tool will not establish eligibility for MassHealth itself. This tool is not an application for any program through MassHealth, and there is no guarantee an individual will be approved by MassHealth for specific services, regardless of the results of this questionnaire. The sole purpose of the questionnaire is to give an individual an idea of the benefits for which it makes sense to apply.

In completing this questionnaire, the term “individual” refers to the person with a disability who needs benefits.
